The State of Vulnerability Remediation in 2026

What goes wrong at 11–50 person tech companies, and how to fix it. Six pages of 2026 data from Verizon, Mandiant, Veracode, Datadog and IBM, plus a 90-day plan you can run without a security team.

The 7 failure patterns behind most missed fixes

Recommended P0–P3 remediation deadlines

A 90-day plan that doubles as SOC 2 and ISO 27001 evidence

What the 2026 data shows

Breaches now start with known

vulnerabilities, and fixes are getting slower

31%

of breaches now start with vulnerability exploitation, up from 20% a year earlier.

26%

of actively exploited vulnerabilities were fully fixed, down from 38%. The median fix time for a critical one is now 43 days.

-7 days

is the mean time to exploit. On average, attacks begin a week before the vulnerability is made public.

The most important fix

Rank vulnerabilities by exploitation, not CVSS score

1. Fix anything in the CISA KEV catalog first

These are confirmed as exploited in the wild. On internet-facing systems, mitigate within 24 hours and fix within 72.

2. Then use EPSS to find what is likely to be exploited next

A high EPSS score on an internal system gets a 7-day deadline.

3. Weigh exposure, and use CVSS to break ties

Internet-facing systems and anything holding customer data move up the queue. Everything else goes on a 30- or 90-day schedule.
